Advocacy Work
Advocacy work is the name that Early Break has given to the 1:1 work we do with young people. Each Early Break client has their own advocacy worker. Below is a description of the process that takes place when a referral is received at Early Break.
After discussing the referral and the young person has agreed to engage with Early Break there is an assesment process that takes place. This involves completing different assessment forms so that the Advocacy Worker can get a really good understanding of the different aspects of the young person’s life and the support needs that they may have.
After completing the assessment paperwork the young person and Advocacy Worker put together something called a care plan. This is basically a plan of what the young person and Advocacy Worker are going to do together. The young person sets themselves some targets in relation to different area’s of their life, e.g substances, school, friendships etc and the care plan sets out exactly what it is that they young person wants to do in relation to these and how the Advocacy Worker is going to support them with this. This may involve different aspects of Early Break such as Alternative Therapies or the Early Break Doctor and could also involve making referrals to other services.
Care plans are reviewed at least every 10-12 weeks so that any progress can be measured and also to make sure there is still a focus to the work that the young person and Advocacy Worker are undertaking.
Each young person’s involvement with Early Break is unique to them and therefore differs in length. Some young people may only work with Early Break for a short period of time whereas others may work with us for months and sometimes even years. The length of time depends on the needs of the young person and their progress towards the goals they set on their care plan.
If, after finishing their work with Early Break a young person feels that they need some substance related support again they can be referred back into Early Break. The young person can request the Advocacy worker that they had previously or can request a different Advocacy worker.
